Students in grades 8-12 participating in any athletic related activity (including cheerleading/dance) are required to have an physical PRIOR to practicing/participating in that activity. A physical form can be printed by clicking this link: Athletic Physical The lower section of the physical labeld "questionnaire" MUST be completed and signed by a parent/guardian and taken to the appointment. Physicals will not be completed if this section is not filled out. The Minden Public Schools/NSAA Parental Consent Form and the Concussion Assessment Form (for grades 9-12 only), which should be completed and returned with your child's physical are also here: Parental Consent Form and Concussion Form.
Kearney County Medical Clinic will be offering athletic physicals from June 1st through August 1st, 2008 for $15.00 to be paid at time of service. After August 1st all physicals will be assessed an office call as well as the cost of the physical. Please call ahead to schedule your physical appointment. Minden Family Practice will be offering free athletic physicals July 28th through August 8th, 2008. All athletic physicals on othe dates will cost $15.00 to be paid at time of service. Students must bring or wear their glasses/contacts (if worn) and be able to give a urine sample at the time of their appointment.
Practice/participation will not be permitted until the school nurse has received this physical. You may turn in a physical for all athletics at the beginning of each school year. Completed forms from Kearney County Medical Clinic and Minden Family Practice will be picked up by the school nurse beginning the week of August 11th 2008 or you can mail them to:
