Parents and caregivers, please review the three forms below. You are not required, but you may choose to review or complete them.

If you choose to complete any of the forms, please turn in the completed form to your student’s school.

Media and Yearbook Release Form

This form is optional but must be completed if you want your student’s photograph to be included in the school’s yearbook.

Directory Information Opt-Out Form

Hmong, Somali, Spanish

Complete this form if you do not want your student’s directory information to be accessible to the public and/or government agencies.

Military Recruiter Release / No Release Form

Hmong, Somali, Spanish

Complete this form if your student is in grade 11 or 12 and you do not want their name and contact information released to military recruiters if requested.
