Early Childhood Screening Registration

START REGISTRATION

Please complete the online registration before your screening appointment.

The online registration takes approximately 5-10 minutes to complete.

Registration Tips:

You will be asked to enter parent/guardian information first, followed by student information for each child being screened.
Be sure to enter the parent or guardian's name and contact information when prompted. Do not enter the child's information until you reach the Student Information section.
If you have a screening appointment for two or more children, select Add Student and enter the information for each child.
Have your child's birth certificate available for upload. If you cannot upload it during registration, please bring it to your screening appointment. (If it was previously uploaded as part of school registration, there is no need to upload it again.)
If you previously completed a screening registration but you were unable to attend the appointment, you do not need to complete a new registration. Please email us at screen@mpls.k12.mn.us so we can locate and use your previous registration record.
