Register with MPS (form below)  

For children new to Minneapolis Public Schools, this allows us to accurately add your child to our database so that we may then register them as a homeschooler. This is a request, not a mandate.

This is not a statute-mandated form, however when we have new-to-MPS students that we need to add to our database (so that we may register your student as a homeschooler), it is highly helpful to know information, such as grade level, so that your student's demographics, contact info, etc., is correct.  This is an MPS form and as such, certain fields are required (although not required by MDE Homeschool statute).  Thank you to those of you that do choose to complete this form as being new to MPS.

Immunization Statement

All new homeschool students must submit a pupil immunization statement (which includes an optional conscientious exemption component). This also applies to returning students who reach the age of 12 years old or enter 7th grade. In lieu of the form, you may send records from your physician's office or MyChart.

If your child is coming from a Minneapolis Public School, we will have your child's immunizations or exemption on file already and there is no need to re-submit. We do not have access to other public districts', charter schools' or private schools' immunization records.

Nonpublic Aid Request

Upon request by Oct. 1 of each year (must be received by 10/1, not postmarked 10/1), via the Nonpublic Aid Request Form, families may receive a certain amount per child, per year to be reimbursed for instructional materials (i.e. textbooks, workbooks) or standardized testing, which is $120.97 for the 24-25 school year.  Please complete the following form and submit to Chrisie.Jennings@mpls.k12.mn.us via email or mail to her attention

Nonpublic Aid Request Form

Families may also request health services (i.e. health screenings, immunization monitoring, etc.) and guidance counseling (gr 7-12), provided through actual services from MPS.

Submit a Reimbursement Request (only available to those who submitted the Nonpublic Aid Request Form by 10/1):

1. Once applicable families are ready to request reimbursement (receipts must be dated between July 1, 2024 and April 25, 2025), please submit the Request for Reimbursement and attach proof of payment for items. Only submit ONE REIMBURSEMENT REQUEST for your allowable total to prevent a delay along with the requirement of becoming a vendor (the vendor set up process has changed this year).  Requests for reimbursement must be received by April 25, 2025.
