Homeschool Services

For all new homeschool students, we must receive a copy of the student's immunizations or a notarized conscientious exemption statement.  Immunizations may be emailed in PDF format (scanned) or mailed to:

You may find immunizations in My Chart (or related) through your physician's office, or, if you immunizations were done solely in Minnesota, you may find them in Docket.

Nonpublic Aid Request

Upon request by Oct. 1 of each year (must be received by 10/1, not postmarked 10/1), via the Nonpublic Aid Request Form, families may receive a certain amount per child, per year to be reimbursed for instructional materials (i.e. textbooks, workbooks) or standardized testing, which is $132.98 for the 25-26 school year.  Please complete the following form and submit to Chrisie.Jennings@mpls.k12.mn.us via email or mail to her attention

The new form for the school year 2026-2027 will be available in August, 2026.

Families may also request health services (i.e. health screenings, immunization monitoring, etc.) and guidance counseling (gr 7-12), provided through actual services from MPS.

Submit a Reimbursement Request (only available to those who submitted the Nonpublic Aid Request Form by 10/1):

1. Once applicable families are ready to request reimbursement (receipts must be dated between July 1, 2025 and April 24, 2026), please submit this Request for Reimbursement and attach proof of payment for items. Only submit one reimbursement request for your allowable total to prevent a delay and avoid the requirement of becoming a vendor.  Requests for reimbursement must be received by April 24, 2026.
